Chemical Peels (from light to deep)

Peels work on a simple premise. A chemical solution is applied to skin that is wrinkled, scarred, sun-damaged, or otherwise pigmented. This solution causes several of the top layers of skin to dry up and to peel off over a period of several days. When the old layer has peeled off, it exposes a new layer of undamaged skin which has a smoother, younger, more even-looking appearance.

Chemical peels are not a substitute for facelifts, but can improve the texture of the skin. Your cosmetic surgeon can discuss your individual circumstances and review the procedure and the peel she plans to use with you. It is important that you understand the procedure, so be sure to ask your surgeon any questions you may still have.

Chemical peels can enhance the appearance of facial and other skin on men and women of all ages. While chemical peeling is available to almost everyone, skin type and coloring are important considerations. The best candidates are usually those with fair, thin skin that has a tendency toward fine wrinkling or scarring on the face. Deeper chemical peels are usually not recommended for highly pigmented skin, such as Asian, black and Mediterranean, or oily, complexions.

This procedure will not prevent aging, however, chemical peels can erase, fade or reduce fine facial wrinkles and remove other surface imperfections. There are now several different types of chemical peels depending upon your particular condition. For certain types of scars and pits, dermabrasion, which uses a tool to “sand” the surface of the skin, may be suggested.

Facial Fillers

Facial Fillers (Restylane®, Juvederm®, Sculptra®) are an injection for the correction of moderate to severe facial wrinkles and folds, such as nasolabial folds, the lines between the nose and mouth. Restylane and Juvederm are hyaluronic acid (HA) fillers. Hyaluronic acid is a natural substance found in all living organisms and provides volume and fullness to the skin.

HA fillers are formulated as a clear gel and uses a dual mechanism of action to correct wrinkles and folds. Upon being injected beneath the skin’s surface, the gel adds natural volume and lift to smooth wrinkles and folds. The gel integrates into dermal tissue then attracts and binds to water molecules to help maintain volume. Restylane is gradually degraded by the body’s own mechanism and disappears without any residue. Results can be seen immediately following treatment and last six months, or even longer.

Sculptra Aesthetic® is a new type of facial injectable made from poly-L-lactic acid, which helps to replace lost collagen. Collagen is a key structural component that keeps skin youthful looking and smooth. As you age, your body’s collagen production decreases, and you may begin to see wrinkles. Sculptra® helps correct shallow to deep facial wrinkles, and folds, that appear with aging by replacing lost collagen. The main ingredient is a biocompatible, biodegradable synthetic material that has been used by physicians for decades.

Within the deep dermis, your skin’s structure is reinforced as Sculptra Aesthetic helps to replace lost collagen. This reinforced collagen structure provides a foundation that gradually restores the look of fullness of your shallow to deep facial wrinkles and folds that has been depleted over time. The result? Sculptra Aesthetic is different because it gradually and subtly corrects these facial wrinkles. This can provide a refined, more youthful looking appearance that can last up to two years.

Sculptra Aesthetic works to replace lost collagen in a series of treatments. On average, three injection sessions are needed over a period of a few months. The number of injection sessions and the number of injections per session vary from person to person.

Botox

Botox® is a form of Botulinum Toxin A manufactured by Allergan, which is purified from bacterial cultures and processed into an injectable protein. It has been used for years to treat patients with certain muscular disorders. More recently, Botox®has been getting attention as an anti-wrinkle treatment that is quick and convenient with minimal side effects.

Wrinkles are formed over time as the facial muscles contract to laugh, smile or frown. As these muscles contract, “grooves” are worn into the skin, resulting in each individuals unique pattern of facial lines. Botox®relaxes the facial muscles by attaching to the nerve terminals, blocking the action of chemical neurotransmitters. This prevents the muscles from contracting, allowing the skin to relax which reduces or even eliminates the overlying wrinkles. Botox®has a long term safety record. For many years, Botox®has been used to help patients who suffer from blepharospasm, a muscular disorder that produces a constant twitching of the eye.

We use a micro needle to inject a small amount of Botox®into the tiny muscles in the area around the wrinkle(s). Pain is minimal and most patients compare the sensation to a bug bite. Injections are normally given every three to six months since the effects of the treatment diminish over time. Usually, results are seen within the first few days following injection. The duration of benefit is often extended with each successive treatment, therefore the second treatment often lasts longer than the first, and so on. Because everyone’s anatomy is a little different, the intensity and duration of Botox®effect varies somewhat from person to person.

Botox®is commonly used on the vertical “frown” lines between the eyebrows and on the horizontal lines on the forehead. It is also quite effective on the “crow’s feet” at the corners of the eyes. Botox®treatments are highly effective, and in most people will create a more alert, refreshed and younger appearance. Often, depending on the lines or wrinkles treated, Botox®can produce a friendlier, less “stern” look, too.

Reasons for considering Botox® Treatment

  • Excessive facial wrinkles.
  • Large wrinkles or furrows in your forehead.
  • Small wrinkles or “crow’s feet” around your eyes.
  • A vertical “frown line” between your eyebrows.

General Procedure

The procedure works by altering the facial muscles beneath the skin that are related to wrinkles. A cream with local anesthetic will be placed on your skin prior to treatment, and ice will be immediately applied upon completion of the Botox® treatment. Typically a small amount (1-2 cc’s) of Botox® is injected into the area of the wrinkle. Botox® works by blocking the impulses from the nerve to the facial muscles, thereby relaxing them. The result is the underlying facial muscles relax and the skin begins to smooth out. The effects of a Botox® treatment will last approximately three to six months after which the patient may choose to repeat the procedure.

Recovery Process

Patients generally return to their normal activities immediately after the procedure, however following the post-procedure guidelines will dramatically reduce the length and intensity of the recovery period. Patients should avoid engaging in heavy physical exercise, applying cosmetics, or laying down flat for a minimum of 4 hours after the injections. It is also advisable to avoid rubbing or massaging the injected area for at least 24 hours.

Eyelid Surgery (Blepharoplasty)

Blepharoplasty is a surgery of the upper or lower eyelid that corrects bulges around the eyes and seeks to remove excess eyelid skin. These conditions can develop from the herniation or bulging of the fat pads around the eyes and from excess, sagging eyelid skin.

It is important not to remove too much of the periorbital fat as this can lead to a hollowed out look. It is also important not to remove too much skin such that the eye is not able to close properly. There are also conditions, such as thryoid disorders, that are treated medically and not surgically.

Blepharoplasty is a corrective procedure that can reduce dark circles or “bags” under the eyes, remove excess wrinkles, and provide a more youthful appearance to the eyes.  One of the first things noticed about a person is their eyes.  During the aging process, wrinkles, lines, and puffiness contribute to a tired or aged appearance.  The blepharoplasty surgery is often performed in conjunction with other facial procedures such as a face lift or facial fat transfer.

Reasons to consider Eyelid Surgery

  • Dark circles or bags under the eyes.
  • Excessive skin or wrinkles surrounding the eyelids.
  • A tired appearance or puffiness due to inherited factors.

General Procedure

The surgical procedure includes tiny incisions made in the natural contours around the eyes which should leave no visible scarring.  The surgeon will usually work on the upper eyelids first to remove any fatty tissue and tighten loose skin.  Lower eyelids are completed next to eliminate puffiness and reduce dark circles.  Combining fat transfer with Blepharoplasty is a beautiful way to completely restore the contours of youth to the eye region.  Newer techniques may include Botox injections to firm fine lines and wrinkles.

The surgery generally takes about one to three hours and is performed in the operating room.  If additional cosmetic procedures are being performed, the eyelid surgery could last longer.

Recovery Process

Normally, post-operative instructions call for plenty of rest and limited movement in order to speed up the healing process and reduce the recovery time.  Patients sometimes report minor pain associated with surgery which can be treated with oral medication.  While complications are rare, patients can minimize the risk for potential problems by carefully following the post-operative directions given after the surgery.  The healing process for this procedure is slightly longer than most other forms of cosmetic surgery; however, the benefits are long-lasting.  Patients will have a more rested and youthful appearance.

Fat Transfer – Grafting

Facial Fat Transfer - Before

Facial Fat Transfer - Before

Most people think that the face ages because of loss of skin elasticity and the effects of gravity over time. This contributes to the appearance of age but loss of volume is as significant, if not more so. This part of the aging process cannot be corrected with a rhytidectomy (facelift) or resurfacing.

The volume lost during aging is not only fat, but involves atrophy of muscle and bone. Transplanted fat can restore the appearance of youthful volume in a way that a face lift can not.

With our technique, autologous adipose (fat) tissue is injected within the muscles of facial expression, thereby restoring fullness following the template of the patients’ own anatomy. Fat is harvested in an atraumatic, sterile manner and fat is then injected into different muscle facial groups. The injections parallel the blood supply, minimizing trauma and optimizing survival of the fat. There are minimal reported complications, down time is minimal, and patient satisfaction is high. Some patients may require two or more treatments for optimal results.

Ideal candidates for this procedure are those with a gaunt look, hollowing eyes or flattened cheeks and lips. It is especially effective for both facelift and post-facelift patients who wish to avoid the pulled appearance of multiple lifts. Fat transfer is also recommended for localized volume loss in areas such as the lips, tear trough, nasolabial folds and chin.

Fat transfer or grafting provides a more predictable, long-term solution to age-related facial defects and successfully restores the smooth, attractive contours of youth.

Avelar Tummy Tuck – is it the right choice for you?

The Avelar, a new “tummy tuck” technique here in the Coachella Valley, is now being offered at the Lombardo Cosmetic Surgery Center in Rancho Mirage.  Developed by Dr. Juarez Avelar in Brazil, this new technique combines liposuction with removal of excess skin and allows for an easier and faster recovery than the traditional procedure.  Traditional “tummy tuck” procedures, known as abdominoplasty, requires extensive surgical dissection and the use of drains for several days to weeks after surgery.

The Avelar is a great alternative for patients who want a “tummy tuck” but have avoided the procedure because of long recovery times, required time off of work and lengthy drainage times.  With the Avelar technique muscles can still be tightened and the navel can be repositioned when necessary. All the advantages of the traditional procedure are present with the added benefit of being able to shape the body and give a more desirable hourglass appearance.  Mommy Makeovers, a combination of The Avelar“tummy tuck” with a breast procedure can also be done.  Flatten the belly and return the breasts to a pre-pregnancy profile.

For more information about all the procedures offered or to make an appointment to discuss whether you are a candidate for this exciting new procedure, call Lombardo Cosmetic Surgery at 760-610-8990 to schedule your complimentary consultation with Dr. Maria Lombardo.

Harvested Fat Ready to Be Injected...

The article below is the latest published research showing that fat grafting/transfer to the breast for enlargement is causing mammographic changes that are indistinguishable from cancer.  There are several surgical procedures available to lift or enlarge the breast that don’t involve autologous fat transfer… These alternatives are safe and viable options. Call or email to make an appointment for your complimentary consultation to discuss the options or any concerns you may have:  760-610-8990 or contact@lombardocosmeticsurgery.com

Brachio Short Before & After

With aging, weight loss, or a combination of the two, the skin of the upper arm often becomes loose and flabby.  Patients are often self-conscious about raising their arms or wearing short sleeves.  An upper arm lift, or brachioplasty, is a procedure that removes excess skin and fat so that the upper arm is more smooth and contoured.  The procedure involves making an incision along the inner aspect of the upper arm from near the elbow to the armpit.  The excess skin is trimmed and the incision closed so that the scars are hidden when the arms are down.  The procedure is often combined with liposuction.  Alternatively, liposuction may be used alone if no skin removal is required.

If a patient has some excess but not enough to warrant a full brachioplasty, he or she may be a candidate for the minor brachioplasty procedure.  The incision is placed in the axilla (armpit) and does not run the entire length down the arm to the elbow.  It will not be as dramatic as the full procedure but is a nice alternative for someone with more modest skin laxity.
